All individuals 12+ are now eligible to register for the COVID-19 vaccine. Please note that Pfizer is approved for ages 12 or older and Moderna/Janssen is approved for ages 18 or older. A parent or guardian must be present at the time of the appointment for anyone under the age of 18. While we will try to honor vaccine type requests, first dose choices may be limited based on vaccine availability.
Third dose boosters of Pfizer are now available for individuals 65 or older who received their second dose at least 6 months before their appointment. At this time, we are unable to provide booster doses for any other population until the CDC releases an official recommendation.
Third doses of mRNA COVID-19 vaccines are available to individuals that qualify who received their second dose at least 28 days prior to their appointment. If you do not qualify using the criteria below but suspect that you will be eligible by the time of the clinic, please register! Until the CDC and FDA have officially approved a date and set a dosing schedule, your registration will be tentative. Currently, CDC is recommending that moderately to severely immunocompromised people receive an additional dose. This includes people who have:
CDC does not recommend additional doses or booster shots for any other population at this time. You can read more about this statement at the CDC website. People should talk to their healthcare provider about their medical condition and whether getting an additional dose is appropriate for them.
- Been receiving active cancer treatment for tumors or cancers of the blood
- Received an organ transplant and are taking medicine to suppress the immune system
- Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
- Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
- Advanced or untreated HIV infection
- Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response
Please be sure to bring your white CDC vaccination card with you to your appointment if you are registering for a second or third dose of any COVID-19 vaccine! If you have already received your first dose with another provider and are due for a follow up, you are welcome to register for your second dose at this location.
Please dress with consideration to having your upper arm exposed. We suggest a short sleeve shirt, if possible. A post-vaccine monitoring period of at least 15 minutes is required.
After registration - download & bring completed consent form to your appointment. Extra consent forms are available at check-in if you do not have access to a printer.
Bring your health insurance card or a clear copy of both sides of the card. If you do not have health insurance you can receive COVID vaccine at no charge. For all others, cash, check, or credit are accepted forms of payment.
