We are able to accept the following insurances:
- All MISD offered plans with BCBS of IL (EPO and HPN) covered at 100%

- Medicare Part B (Flu vaccines only-please provide a copy of any supplement plans also)
REMEMBER ALL BCBS TRS CARDS ARE NOW INVALID FOR MISD EMPLOYEES. Don't forget to present your current insurance card. Temporary cards can be printed at www.myblueelementil.com
We do not accept HMO plans.
*We are unable to accept Aetna & Blue Cross Gold, Silver & Bronze plans and Cigna Local Plus. Please see the consent forms for other known exceptions of plans that do not cover vaccines at 100%.
A paper copy of your current insurance card and driver’s license are required. Please bring your paper copies with you when you receive your vaccines. Electronic cards cannot be accepted.
After you register you will see a confirmation page containing a link to a consent form. You only need to print out one consent form for vaccines. B12 requires an additional form. Please bring these filled out along with your photo copies. This will ensure the shortest possible wait. Registration times are requests only and you should still expect a short wait during the clinic. Arriving at the clinic with your photocopies and your consent form already filled out will help keep the wait times to a minimum.
If you received routine vaccinations from our company in the past, other than flu, and need a copy of your records, please email us at info@stayinghealthymedical.com. In the email you will need to include your name and birth date. Please do this at least a week before your scheduled clinic to ensure we have ample time send you your records.
The COVID-19 vaccine will be available for participants age 18 and up.. The most current formula available at the time of your clinic will be offered and CDC guidelines for doses and administration will be followed.
For individuals age 65 and over, only flu vaccines can be administered if using insurance. Age appropriate vaccines can be administered to those 65 and over that are paying out of pocket for the vaccine.
Depending on the carrier, insurance claims may be filed as Community Health and Immunization Services or Staying Healthy Medical Services.
