Flu Shots for Granite Properties
Granite Properties - Plano will be offering a vaccination clinic Wednesday September 18, 2024. Please follow the steps below to register We are able to accept the following PPO insurances: • Aetna • Blue Cross Blue Shield • Cigna • Humana • United HealthCare • Medicare Part B (Flu Only-provide cards for any supplement plans also) We are unable to accept HMO plans. We are unable to accept Aetna and Blue Cross Gold, Silver or Bronze plans and Cigna Local Plus Plans. Most plans cover immunizations at 100%. Please see the consent form for known exceptions. Only flu can be given to individuals age 65 and over that are using insurance. Age appropriate vaccines can be administered to those 65 and over that are paying out of pocket for the vaccine. If you are using insurance for your immunizations, you must bring a photocopy of your insurance card and a photocopy of your driver’s license to the clinic. Electronic insurance cards cannot be accepted. A paper copy must be provided. After you register you will see a confirmation page containing a link to a consent form. You only need one consent form for vaccines. B12 requires an additional form. Please print and complete the forms prior to the clinic. They will need to be given to the nurse along with your photocopies. This will ensure the shortest possible wait. (Registration times are requests only and you should still expect a short wait during the clinic.) The COVID-19 vaccine will be available for participants age 18 and up for primary doses or booster doses. The most current formula available at the time of your clinic will be offered and CDC guidelines for doses and administration will be followed. If you register for a time that ends up not being convenient for you there is no need to cancel or re-register. Please attend the clinic at a time that works best. If you would like to add or remove vaccines please discuss this with the nurse during the clinic. Your registration does not need to be changed. To cancel the appointment completely, please use the link at the bottom of your confirmation email. The nurse will have flu vaccine appropriate for ages 6 months and up. Please check with your clinic organizer if family members may attend or if a specialty flu vaccine is needed, such as High Dose or Preservative free. If you are not using insurance the cost of the Flu vaccine is $35 payable to Staying Healthy Medical Services. Vitamin B12 will be available for $25 cash or zelle. Depending on the carrier, insurance claims may be filed as Staying Healthy Medical Services or Community Health and Immunization Services. Thank you!

I authorize Passport Health to disclose my health information to my state immunization registry, my physician, my employer and my insurance company. I authorize Passport Health to notify me of future health events, services or products. I hereby acknowledge receipt of Passport Health's Notice of Privacy Practices. If I am signing this consent on behalf of another individual, I hereby certify that I have the legal right to do so as the person's parent or legal guardian, pursuant to a health care power of attorney, pursuant to a court order, or pursuant to some other legal right to consent to health care for the individual. I agree that the insurer or payer listed above is my primary medical coverage and that I am responsible for payment if services are not covered for any reason or are subject to any co-pays, deductibles, coinsurance or prior authorizations.
