Vaccination clinics are open to Dallas College students, employees and employee's dependents,
Passport Health/Staying Healthy Medical Services can accept the following insurances:
- ALL plans offered to Dallas College employees through Blue Cross Blue Shield- cover the vaccines at 100% - A PCP must be listed on JEA cards otherwise insurance cannot be accepted
- Aetna*
- Blue Cross Blue Shield*
- Cigna *
- Humana
- United Health Care
- Medicare Part B (Flu only. Please also provide a copy of any supplemental plans.)
We do not accept HMO plans.
*We are unable to accept Aetna & Blue Cross Gold, Silver & Bronze plans and Cigna Local Plus. Please see the consent forms for other known exceptions of plans that do not cover vaccines at 100%.
What to Bring with You:
1. A paper copy of your current insurance card and driver’s license are required. Please bring your paper copies with you when you receive your vaccines. Electronic cards cannot be accepted.
2. After you register you will see a confirmation page containing a link to a consent form. You only need to print out one consent form for vaccines. B12 requires an additional form. If possible, please bring these forms filled out along with your photo copies. This will ensure the shortest possible wait. Registration times are requests only and you should still expect a short wait at the clinic. Arriving with your photocopies and your consent form already filled out will help keep the wait times to a minimum.
How to Request Records:
If you received routine vaccinations from Passport Health/Staying Healthy Medical Services in the past, other than flu, and need a copy of your records, please email info@stayinghealthymedical.com. In the email you will need to include your name and birth date. Please do this at least a week before your scheduled clinic to ensure we have ample time to send you your records.
Age Requirements for Certain Vaccines
The newest COVID-19 vaccine will be available for participants age 18 and up. Please note that there is a limited supply of this vaccine. Participants are welcome to request it, but it is not guaranteed.
The new Shingles vaccine (Shingrix) will be available for participants ages 50-64 only if using insurance. Please note that this vaccine is known to cause more frequent local reactions than other routine immunizations.
For individuals age 65 and older, only flu vaccines can be administered if using insurance. Age appropriate vaccines can be administered to those 65 and older that are paying out of pocket for the vaccine.
Depending on the carrier, insurance claims may be filed as Community Health and Immunization Services or Staying Healthy Medical Services.
